Old Laryngospasm Complication: New Treatments

چکیده
Laryngospasm (LEP) is defined as the sustained closure of the vocal cords. It is a primitive protective airway reflex (VA), which occurs to safeguard the integrity of the airway by protecting it from tracheobronchial aspiration [1]. LEP is also defined as an exaggerated response to closure reflex or glottic muscle spasm. Essentially it is a protective reflex, which acts to prevent the entry of any foreign material into the tracheobronchial tree. The presence of this reflex results in an impediment to adequate breathing, under these conditions it becomes a sudden obstruction of upper VA. A feature of LEP is that VA closure is maintained even after the initial causal stimulus disappears. In any of the situations mentioned above, we face a partial or total loss of VA, and therefore an anesthetic urgency. 40% of VA obstructions are secondary to LEP, may result in a lifethreatening complication, and is a major cause of cardiac arrest in the pediatric patient.

Early airway complication after deep and awake tracheal extubation in children less than 4 year old referring for non-pharyndeal surgeries to Kerman university hospitals
In this clinical trial 100 children less than 4 years old,who had underwent non-pharyngeal surgeries in Kerman university hospitals were selected randomly and investigated for early airway complications after tracheal extubation.patients were divided into two groups:the first group(n=50) were extubated while they were still anesthetised and the second group(n=50) were extubated after regaining...
متن کاملLaryngospasm as a Cause of Unsuccessful Placement of Laryngeal Mask Airway ProSeal: A Case Report.
Laryngospasm is a potential complication encountered during anesthesia using a laryngeal mask airway (LMA). We report a case in which laryngospasm resulted in unsuccessful placement of an LMA ProSeal Airway (Teleflex Inc), and we discuss the various causes of unsuccessful placement of this type of airway device. Laryngospasm causes increased resistance to gas flow by inducing closure of vocal c...
متن کاملDiagnosis and outcome of SCN4A-related severe neonatal episodic laryngospasm (SNEL): 2 new cases.
Mutations of SCN4A encoding the skeletal muscle sodium channel Nav 1.4 cause several types of disease, including sodium channel myotonias. The latter may be responsible for neonatal symptoms, including severe neonatal episodic laryngospasm (SNEL). Establishing the diagnosis of SCN4A-related SNEL early in the neonatal period is crucial because treatment is available that can reduce laryngospasm ...
متن کامل[Low dose propofol vs. lidocaine for relief of resistant post-extubation laryngospasm in the obstetric patient].
BACKGROUND Post-extubation laryngospasm is a dangerous complication that should be managed promptly. Standard measures were described for its management. We aimed to compare the efficacy of propofol (0.5mg.kg-1) vs. lidocaine (1.5mg.kg-1) for treatment of resistant post-extubation laryngospasm in the obstetric patients, after failure of the standard measures. METHOD This study was conducted o...
متن کاملLaryngospasm-induced pulmonary edema.
Laryngospasm is one of the more common forms of airway obstruction encountered by an anesthetist. Therapy usually is straightforward, with resolution of the obstruction normally occurring within minutes. In some cases, however, the patient's vigorous inspiratory efforts may lead to a rapidly deteriorating form of pulmonary edema. Two cases are presented that are quite typical of the development...
